A total of three DEMATEL matrices (L2 - L1; L3 - L1; and L4 - L1) were included in the form, with latent conditions at HFACS levels 2, 3, and 4 set across three respective matrices against the five active failure categories (DE, SBE, PE, RV, EV) at HFACS level 1. > Six subject matter experts including one state-level chief investigator of accidents, two academics in air safety management and accident investigation, one regulatory safety management expert, one active airline pilot involved in flight and cabin crew safety training development, and one airline pilot involved in industrial and organisational psychology, were recruited by convenience sampling.
